Responsibilities
- Oversee remote patient monitoring services for individuals with chronic illnesses.
- Collaborate with Family Medicine Care Coordinators and interdisciplinary teams.
- Develop and implement personalized health care plans for patients.
- Facilitate effective communication between patients and health care providers.
- Monitor patient progress and adjust care plans as necessary.
- Provide education and resources to patients and their families.
- Document and maintain accurate patient records and data.
- Ensure compliance with healthcare regulations and standards.
Requirements
- Bachelor’s degree in Health Education or related field, or California LVN license.
- Experience in a healthcare setting preferred.
- Strong written and verbal communication skills.
- Proficient in Microsoft Office and electronic health record systems.
- Ability to work independently and as part of a team.
- Excellent organizational skills and attention to detail.
- Fluency in English and Spanish is a plus.
- Ability to manage confidential information with discretion.
