Key Responsibilities

Care Coordination & Support

  • Support patients in navigating healthcare and social service systems,

including assistance with applications, forms, and referrals.

  • Identify barriers such as transportation, insurance, food insecurity, and

housing, and connect patients to resources.

  • Provide follow-up contacts to ensure continuity of care and reinforce patient

engagement. Mobile Clinic Operations

  • Build trusting relationships with community members to increase awareness and

utilization of mobile clinic services.

  • Assist in setting up and closing mobile clinic sites
  • Help guide patients through clinical flow including check-in, navigation, and

follow-up scheduling.

  • Work collaboratively with medical, behavioral health, and administrative team

members to support daily operations. Health Education & Advocacy

  • Deliver health education under supervision on topics such as preventive care,

chronic disease management, nutrition, and wellness.

  • Advocate for patients by communicating needs and concerns to healthcare

providers and social service agencies.

  • Participate in community events, screenings, and health fairs as part of the

mobile health team. Documentation & Reporting

  • Learn and maintain accurate documentation of patient interactions,

screenings, and referrals in the electronic health record (EHR).

  • Uphold confidentiality and compliance with HIPAA and organizational policies.
  • Contribute to data collection for program reporting and evaluation.

Training & Apprenticeship

  • Participation in the Pennsylvania AHEC Community Health Worker Training

Program is required.

  • Apprentices will complete both classroom-based instruction (virtual &

in-person) and on-the-job supervised training.

  • The apprenticeship provides a pathway to certification as a Pennsylvania

Certified Community Health Worker (CCHW).

  • Ongoing mentorship, professional development, and evaluation will be provided

throughout the apprenticeship period.

Qualifications

  • Minimum high school diploma or equivalent required; some college coursework

in health or human services preferred.

  • Interest in pursuing a career in healthcare, social services, or community

advocacy.

  • Strong communication and interpersonal skills with the ability to build trust

and rapport.

  • Commitment to cultural humility and working with diverse populations.
  • Ability to work both independently and as part of the care team.
  • Willingness to travel with the mobile unit and work in community-based

settings.

  • Valid driver’s license required.

Work Environment

  • This role involves working in a mobile health clinic environment, community

settings, both indoors and outdoors. May include visits to patient homes as needed.

  • Flexible schedule may include evenings or weekends, depending on community

needs.

  • Physical requirements include setting up equipment, engaging in outreach
